Goal - Improve the health, safety, and well-being of lesbian, gay,
bisexual, and transgender (LGBT) individuals. - Overview - LGBT
individuals encompass all races and ethnicities, religions, and social
classes. Sexual orientation and gender identity questions are not asked
on most national or State surveys, making it difficult to estimate the
number of LGBT individuals and their health needs. Research suggests
that LGBT individuals face health disparities linked to societal
stigma, discrimination, and denial of their civil and human rights.
Discrimination against LGBT persons has been associated with high rates
of psychiatric disorders,1 substance abuse,2, 3 and suicide.4
Experiences of violence and victimization are frequent for LGBT
individuals, and have long-lasting effects on the individual and the
community.5 Personal, family, and social acceptance of
sexual orientation and gender identity affects the mental health and
personal safety of LGBT individuals.6.

So, if discrimination related to sexual orientation is directly
connected to mental health problems for some MSM, then how well do
mental health services address this issue?... . Further some
participants were unable to disclose their sexuality in mental health
services even though they considered it central to their mental health
problems. This was the case for the two youngest men interviewed.
Daniel explained how a medical and symptom based approach was a barrier
to discussing his issues regarding his sexuality... They reported that
staff generally do not ask clients about sexual orientation and they
gave various reasons for this. One barrier to asking about sexuality is
the perceived dominance of the medical model... This focus on ‘mental
illness’ reduces the chance that staff may see conversations about
sexual orientation as important or relevant to their work. Instead, the
medical model requires them to focus primarily on symptoms and
diagnosis... Yet, homosexuality is marginalised, often remaining
unacknowledged. Thus, when homosexuality is omitted for MSM it is not
just their sexual behaviour that is ignored; rather, many other aspects
of their lives are also ignored such as their relationships, important

Homoignorance: considerations for the gay or lesbian client in therapy
(2011): This article explores the issues that arise when working with
lesbian and gay clients in therapy. Fortunately, explicit homophobia is
now frowned upon in almost all therapeutic circles, but I’m looking at
the ways in which even ‘gay-friendly’ therapists can unconsciously
damage clients by ignoring crucial differences between gay and
heterosexual lives. This unconscious ‘homo-ignorance’, as it has been
called, is not limited to heterosexual therapists. I’ve been powerfully
struck while I was researching this piece how much it also applies to me
as an openly gay man working in a liberal, integrative training
institute. And I believe this is an important area of consideration
because the figures show that we as therapists will encounter a
disproportionate number of lesbian and gay clients in our work...

What to Say When Your Teenager Says She's Gay
(2010): What role, if any, should parents and schools play in a child's
emerging sexual orientation? Sparks have been flying around that
question this spring.
Early last month, a small group called the American College of
Pediatricians (ACP) sent a letter to the nearly 15,000 school
superintendents in the U.S., stating that most adolescents who
experience same-sex attraction at age 12 no longer do by age 25, and
warning that prematurely labeling them could lead some "into harmful
homosexual behaviors they otherwise would not pursue." The letter also
stated that homosexual attraction and/or gender confusion "can respond
well to therapy." The far larger American Academy of Pediatrics (AAP)
posted a statement saying it is in no way affiliated with the ACP and
referred schools and parents to its own publications that urge
acceptance of gay, lesbian and bisexual youth... Other incidents this
spring: One Mississippi high school canceled its prom rather than allow
one senior to bring a same-sex date; another refused to let a girl be
photographed for the yearbook wearing a tuxedo rather than the
customary formal drape, and a group called PFOX (Parents and Friends of
Ex-Gays) distributed fliers advocating sexual-reorientation therapy in
some Montgomery...

Bulimia Sufferers Often Gay, Bisexual, a New Study Finds: "135
men with eating disorders who received treatment at Massachusetts General
Hospital in Boston. Of the 135 men, 62 (46%) had bulimia, and 42% of the
bulimic men either were homosexual or bisexual. Another 30 men (22%) had
anorexia nervosa. Fifty-eight percent of the anorexic patients were identified
as asexual, which the investigators defined as lacking all sexual interest
for one year prior to being evaluated for the study. Overall, 27% of subjects
were homosexual or bisexual, and 32% were asexual. (Study

Download. There is a growing body of evidence, both international
and Australian, to suggest that non-heterosexual people experience
anxiety and depression at higher rates than their heterosexual peers and
are at greater risk of suicide and self-harm. Yet, there has been
little recognition of the implications of this for policy and practice.
Contested debates on the influence of genes on sexual orientation aside,
non-heterosexual people are not physiologically different from their
heterosexual peers to all intents and purposes. However, they are likely
to have different life experiences that contribute to at least two
areas of particular need in relation to mental health and depression:
managing the effects of homophobia and its consequences on a daily basis
through the life course; and gaining access to mental health services
that are able to respond to their needs in socially and culturally
competent ways. The invisibility of these issues prompted the
partnership between beyondblue and the Australian Research Centre in
Sex, Health and Society (ARCSHS) at La Trobe University to undertake a
systematic and targeted international literature review of the evidence
base on depression and related issues among non-heterosexual people,
published between the years 2000 and 2008.

homosexual women as masculine. I evidence this through an examination of
published psychoanalytic case histories of female homosexuals from 1920
to the present day. Informed by Foucault‘s genealogical methodology, I
propose that this characterisation constitutes an ―unconscious rule‖,
which transcends the differences between the various schools of
psychoanalysis, and which has remained constant throughout its history
and impervious to the challenges and critiques of its theory and
practice. Since the late 1980s, the most recent critical engagement with
psychoanalysis has come from queer theory. I argue that, despite the
apparent promise of this engagement, queer theory, like psychoanalysis,
is subjected to the same ―rule‖: lesbians are masculine.
Disparities in Health-Related Quality of Life: A Comparison of Lesbians and Bisexual Women
(2009): We investigated the association of health-related quality of
life (HRQOL) with sexual orientation among lesbians and bisexual women
and compared the predictors of HRQOL between the 2 groups... We used
multivariate logistic regression to analyze Washington State Behavioral
Risk Factor Surveillance System population-based data (2003 to 2007) in a
sample of 1496 lesbians and bisexual women and examined determinants of
HRQOL among lesbians and bisexual women.. For lesbians and bisexual
women, frequent mental distress and poor general health were associated
with poverty and lack of exercise; poor general health was associated
with obesity and mental distress. Bisexual women showed a higher
likelihood of frequent mental distress and poor general health than did
lesbians. The odds of mental distress were higher for bisexual women
living in urban areas as compared with nonurban areas. Lesbians had an
elevated risk of poor general health and mental distress during
midlife... Despite the standard practice of collapsing sexual minority
women into a single group, lesbian and bisexual women in this study
emerge as distinct groups that merit specific attention. Bisexual women
are at elevated risk for poor HRQOL.

Download.This study discusses the impact of stress specific to being
lesbian, gay, or bisexual (LGB), measured by means of the concepts of
stigma consciousness and internalized homonegativity, on the mental
well-being of LGB youth. Also, the effects of positive and negative
social support were considered within the model. The sample consisted of
743 LGBs less than 26 years old who were recruited during the online
ZZZIP survey in Flanders, Belgium. Hierarchical regression shows that
LGB-specific unsupportive social interactions have the greatest direct
effect on mental well-being of LGB youth, followed respectively by
stigma consciousness, internalized homonegativity, and confidant
support.

Download. Many gay and bisexual men struggle with unique issues
related to being both a man and a sexual minority person. The purpose of
this study was to use feminist theory to test two mediation models
examining the roles of both gender role conflict and internalized
heterosexism (IH) in gay and bisexual men’s psychological distress.
Findings from the best fitting model revealed that gender role conflict
was both directly and indirectly (through IH) related to self-esteem,
and self-esteem was directly and indirectly (through avoidant coping)
related to psychological distress. Research and practice implications are discussed.

Download. Dating in adolescence plays an integral part in the
development of sexual and social identities. This process is
particularly salient for sexual minority youth who face additional
obstacles to their identity formation due to their marginalized status.
We investigated the influence of participating in a same-sex
relationship (SSR) or an opposite-sex relationship (OSR) on sexual
minority youths' psychological well-being (i.e., symptoms of depression,
anxiety and internalized homophobia, and self-esteem) in an
ethnically-diverse sample of 350 youth (55% male) between the ages of
15-19 years, recruited from three GLBT drop-in centers in the New York
City area. Using longitudinal data, we examined youths' SSR and OSR over
time. Multivariate regression analyses suggest that involvement in a
SSR was positively associated with changes in self-esteem in males, and
negatively correlated with changes in internalized homophobia in
females. We discuss the implications for positive development in sexual
minority adolescent populations.

Abstract. Professional psychologists who work with gay men have
noted that traditional masculine ideals play a prominent role in the gay
community whereby some endorse these traditional ideals and stigmatize
effeminate behavior by other gay men. One hypothesis is that this
behavior reflects negative feelings about being gay. This article
examined this hypothesis by reporting the results of an online survey of
622 self-identified gay men. Participants completed the Gender Role
Conflict Scale, Lesbian and Gay Identity Scale, the Social Desirability
Scale, and questions related to the importance of masculinity. Results
showed that most participants valued the public appearance of
masculinity; and they ideally wished to be more masculine than they felt
they were (Cohen's d = 0.42). A multiple regression analysis showed
that the degree to which they valued masculinity and were concerned with
violating masculine ideals was positively related with negative
feelings about being gay (Cohen's f(2) = .67). These findings highlight
the importance of exploring the role that masculine ideals play in gay
client's lives given that negative feelings about oneself can adversely
affect psychological well-being.

Download. In the present study, the authors investigate associations
between sexual orientation and mental health/substance use morbidity
using information obtained from 2,272 individuals, including 652 sexual
orientation minorities, age 18 to 72 years, interviewed in the
California Quality of Life Survey. Results confirm that minority sexual
orientation is a risk indicator for psychiatric morbidity. However,
levels of increased risk vary within this subpopulation by both gender
and patterns of sexual orientation expression. Among gay/bisexual men,
much of this greater burden is related to concurrent HIV infection.
Reducing excess mental health morbidity risk among sexual orientation
minorities could result in possibly a 5% to 11% reduction in the burden
of the disorders assessed here among the adult population. Sexual
orientation represents an important, but relatively understudied,
individual characteristic shaping risk for psychiatric morbidity.

Download. In multivariable models adjusting for demographics, WSW were
disproportionately more likely to have mental health and psychosocial
issues noted in their medical records, including: a clinical diagnosis
of depression, anxiety, and posttraumatic stress disorder, history of
suicide attempts, and inpatient psychiatric/mental health treatment.
However, WSW were significantly less likely than WSM to engage in "high
risk" HIV/STD sexual behavior. In a final multivariable model, same sex
behavior was not associated with a different likelihood of being
diagnosed with an STD, compared with opposite sex behavior. However, WSW
diagnosed with STDs were at increased odds of having bipolar disorder
and utilizing outpatient mental health counseling services compared with
WSW without STDs. WSW with a history of STDs were at increased odds of
having attempted suicide in the past, utilizing both outpatient and
inpatient mental health treatment services, and having a history of
injection drug use compared with WSW without a history of STDs.

Download.Previous research has demonstrated that people who report
same-sex experience tend to have poorer mental health than heterosexual
people in adolescence and adulthood. Yet, little is known about how
same-sex experience is associated with changes in mental health between
the two life stages. Using data from the National Longitudinal Study of
Adolescent Health (n = 12,056), this study assesses patterns of same-sex
experience between adolescence and young adulthood and their
consequences for changes in depressive symptoms, binge drinking, and
drug use. Compared to people with no same-sex experience, those who
report such experience only in adolescence show greater levels of mental
health problems in adolescence, but they do not show any worse mental
health changes during the transition into young adulthood. People who
develop their first same-sex experience in young adulthood, however,
tend to show worse changes, consistent with the argument that entry into
a stigmatized role is psychologically harmful. Among females, those who
report same-sex experience in both life stages also show worse mental
health changes, indicating that the continuity in minority status
contributes to their cumulative disadvantage. However, these differences
are modest, and substantial amounts of variations in mental health
changes are observed within each group. Findings are used to address the
dynamic aspect of mental health disparity linked to sexuality.
High Levels of Psychological Distress in Men Who Have Sex with Men Are Independent of HIV Status
(2011): This study assessed psychological distress (PD) in men who have
sex with men (MSM) accessing primary health clinics in Australia.
Relationships between PD, HIV status and substance use were explored. A
cross-sectional convenience sample of 250 MSM completed the Personality
Assessment Screener (PAS). One-third (n = 83) scored in the PAS
clinically significant range, suggesting significant mental health
symptoms. Negative Affect (27 per cent clinically significant), Suicidal
Thinking (29 per cent clinically significant) and Amphetamine use
significantly positively correlated with PD. There were no significant
differences between HIV diagnostic groups on PD. A third of MSM
displayed PD. Psychological screening may provide valuable information
for improving the psychological well-being of MSM, regardless of their
HIV status.
Mental health of the non-heterosexual population of England
(2011): The Adult Psychiatric Morbidity Survey 2007 (n = 7403) was
representative of the population living in private UK households...
Self-reported identification as non-heterosexual (determined by both
orientation and sexual partnership, separately) was associated with
unhappiness, neurotic disorders overall, depressive episodes,
generalised anxiety disorder, obsessive–compulsive disorder, phobic
disorder, probable psychosis, suicidal thoughts and acts, self-harm and
alcohol and drug dependence. Mental health-related general practitioner
consultations and community care service use over the previous year were
also elevated. In the non-heterosexual group, discrimination on the
grounds of sexual orientation predicted certain neurotic disorder
outcomes, even after adjustment for potentially confounding demographic
variables... This study corroborates international findings that people
of non-heterosexual orientation report elevated levels of mental health
problems and service usage, and it lends further support to the
suggestion that perceived discrimination may act as a social stressor in
the genesis of mental health problems in this population.

contexts and the transnational influence of the US-based stigma model of
LGBT psychology. The challenges and possibilities posed by
internationalism are discussed with reference to the dominance of the
United States, the cultural limits of terms such as “lesbian, gay,
bisexual and transgender”, intergenerational communication between
researchers, and the role of events such as the Summer Institute in
creating an international community of LGBT psychologists.

We attempted to estimate the influence of minority stress and familial
factors on psychiatric disorder among adults with same-sex sexual
partners. Self-report data from a 2005 survey of adults (age 20-47
years, n=17,379) in the population-based Swedish Twin Registry were
analysed with regression modelling and co-twin control methodology...
Rates of depression, generalized anxiety disorder (GAD), eating
disorders, alcohol dependence and attention deficit hyperactivity
disorder (ADHD) were increased among men and women with same-sex sexual
experiences. Adjusting for perceived discrimination and hate crime
victimization lowered this risk whereas controlling for familial
(genetic or environmental) factors in within-twin pair comparisons
further reduced or eliminated it... Components of minority stress
influence the risk of psychiatric ill-health among individuals with any
same-sex sexual partner. However, substantial confounding by familial
factors suggests a common genetic and/or environmental liability for
same-sex sexual behaviour and psychiatric morbidity.

Download. From the 2005–2007 New York City Youth Risk Behavior
Surveys, 3805 male and 3456 female adolescents reported having had sex
and the sex of their sexual contacts. Subgroups were constructed: only
opposite-sex partners; only same-sex partners; and partners of both
sexes (both-sex partners). Weighted prevalence, risk behaviors (eg,
using drugs/alcohol with sex), and sexual identity among subgroups were
described. Similar numbers of sexually active male (3.2%) and female
adolescents (3.2%) reported only same-sex behavior, but fewer male than
female adolescents reported both-sex partners (3.7% vs 8.7%; P <
.001). Male adolescents with both-sex partners reported a higher
prevalence of sexual risk behaviors than male adolescents with only
opposite-sex or only same-sex partners. Female adolescents with both-sex
or only same-sex partners reported a higher prevalence of risk
behaviors than female adolescents with only opposite-sex partners.
Adolescents with both-sex partners reported a marked prevalence of
dating violence and forced sex. Many adolescents with only same- or
both-sex partners (38.9%) self-identified as straight.

The U.S. military invests significant resources into support for
servicemembers and their families. Due in part to the federal policies
known as “Don't ask, don't tell” (DADT) and The Defense of Marriage Act
(DOMA), the military does not provide this support to lesbian military
families. This study researched the potential of lesbian-focused
community mental health centers as viable alternative providers for
these servicemembers. The findings of the study showed that the best
prepared lesbian-focused mental health centers can provide only 73% of
the resources which the military states are needed. The average center
had access to only 40% of the defined resources. The study raises new
questions about community based mental health centers‟ ability to treat
military families and the potential value of research on “officially
invisible” populations.

model to help heterosexual therapists become more aware of the influence
of their own heteronormative assumptions, heterosexual privileges, and
heterosexual identities on the therapy process. This article also
provides definitions of concepts central to the practice of affirmative

therapists and clients can use to deconstruct heterosexism in the
context of therapy.

Our results contradicted this idea, with nonheterosexual men and women
scoring significantly higher on Neuroticism and Psychoticism than their
heterosexual counterparts, suggesting an overall elevation of
psychiatric risk in nonheterosexuals. Secondly, we used our genetically
informative sample to assess the viability of explanations invoking a
common cause of both nonheterosexuality and psychiatric vulnerability.
We found significant genetic correlation between sexual orientation and
both Neuroticism and Psychoticism, but no corresponding environmental
correlations, suggesting that if there is a common cause of both
nonheterosexuality and psychiatric vulnerability it is likely to have a
genetic basis rather than an environmental basis.

past-year prevalence of major depression and generalized anxiety
disorder was lower among LGB respondents living in states with higher
concentrations of same-sex couples, compared with LGB respondents in
states with lower concentrations. Additionally, the increased risk for
mood and anxiety disorders among LGB individuals exposed to economic
adversity and social isolation was evident only in states with low
concentrations of same-sex couples. These interactions between the risk
and protective factors were not found among heterosexuals, suggesting
specificity of the effects to LGB individuals. Results were not
attenuated after controlling for socio-demographic factors, state-level
income inequality, state-level policies targeting LGBs and state-level
attitudes towards LGB-relevant issues... These results provide evidence
for the protective effect of social/contextual influences on the
prevalence of psychiatric disorders in LGB individuals. Measures of the
social environment should be incorporated into future research on the
mental health of LGB populations.

Download. This aim of this study is to examine predictors of
specific motivations for engaging in cutting behavior among a community
sample of sexual minority youth. The study involved secondary analysis
of data collected by a community-based organization serving lesbian,
gay, bisexual, and transgender (LGBT) youth and their allies.
Descriptive and logistic regression analyses were conducted using a
final sample of 131 sexual minority youth ages 13–24. Analyses indicate
that cutting occurs at high rates among sexual minority youth and that
certain demographic characteristics, psychosocial variables, and mental
health issues significantly predict endorsement of particular
motivations for cutting among youth in this sample. Implications for
social work assessment and intervention with sexual minority youth are
discussed.

The stereotypical portrayals of Queers in Hollywood film do not provide
such models. Historically, in many films the Queer character, who is
rarely a hero, ends up dead before the end of the film; sometimes at the
hand of another, or from disease but most often by his her own hand.
Sources of Data: Questionnaires were distributed to 35 adult individuals
primarily, within the greater Sacramento area, 27 were returned and
analyzed. In addition, five participants were contacted requesting
supplement dialogue. Three individuals were able to participate in the
additional conversations. Both the questionnaires and the dialogue
focused on the influence the participant felt film had in their identity
development as Queer people. Although the concern of this study is the
prevalence of Queer youth suicide, adults were used in this study due to
the problems associated in finding an adequate sample of Queer youth.
The adults were asked to reflect on early films viewed and the feeling
generated by those films. Conclusions Reached: The responses from the
participants while not conclusive, build a strong case for the negative
effect seeing demeaning and deadly portrayals of Queer people in film
had on the self-image of these Queer respondents.

The medical industrial complex has historically contributed to the
oppression and pathology of queer and disabled people in a myriad of
ways; through forced medication and institutionalization, as well as
denial of self-determination and identity. Queer theory and disability
theory both challenge modes of normalcy directly related to both queer
and disabled identities by using a lens that encompasses sexuality,
gender, embodiment, health and impairment. This paper will use queer
theory and disability theory to analyze the ways in which queer and
disabled identities are connected and co-constructed, as well as, the
ways in which these intersections may expand our thinking in social
work. The current diagnosis and controversy around Gender Identity
Disorder as a diagnosis in the DSM will be examined through the lenses
of queer theory and disability theory, to aid in the continued movement
to depathologize queer and disabled identities. It is the perspective of
this paper that the liberation of queer people is directly linked to
the liberation of disabled people and that trans/queer justice is
disability justice.

The findings of this study have implications for clinical and
counseling practice. Before proceeding to these implications, I want to
note that the mere positing of these implications based on the findings
drawn from the lives examined for this dissertation does in no way
suggest that the young men from whom so much was learned are in need of
or require therapeutic intervention. The heart of the therapeutic
process is the relationship between the patient and therapist. The
present effort suggests that authenticity, mutuality, trust, and empathy
should be nurtured within the therapeutic process. Both the therapist
and patient must develop trust for one another and for their
relationship. At times, patients typically approach therapy with fear
and trepidation, sometimes engendered by past clinical relationship
characterized by hurt, invalidation, and even pain. To help nurture
trust from the patient, it is important for that the therapist to learn
to trust the patient. To do this the therapist must forego any semblance
of the all knowing, all-powerful therapist. Mutuality does not mean
equality-it means equity and openness to change and healing on both
sides. While the therapist certainly must exercise certain authority
within the therapeutic setting and the patient moves into a place of
vulnerability, the attitude should be one of empowerment and not what of
power over. Therapist and patient alike must risk change and
uncertainty. Based on the previous discussion, the following implication
for clinical and counseling practice can be derived from the findings

the Challenges of Being Gay or Lesbian: A first rate series
of videotapes aimed at helping psychotherapists work with gay and
lesbian clients; it also provides the lay person with penetrating insights

National Suicide Prevention LGBT Project:
PACE launched its national review of mental health support for LGBT
people on August 17th. Commissioned by the National Mental Health
Development Unit at the Department of Health the research will examine
what is currently being done to promote better mental health for LGBT
people in England.

making its submission to the Because Mental Health Matters consultation
paper wants to make a clear statement that the most effective
prevention strategy to mental health problems in GLBTI communities
would be to minimise and eliminate discrimination on the basis of
sexual orientation and gender identity. To do this would require a
significant whole of government commitment and approach but in the
context of implementation of the Charter of Human Rights and the
government’s response to the imminent release of the Equal Opportunity
